FIRST DOSE - COVID-19 VACCINATION ON WEDNESDAY 27 OCTOBER 2021

 

Students who opted to be vaccinated  but who missed the first dose of Covid-19 vaccination are kindly requested to be present at school on Wednesday 27 October 2021 if they wish to have their first dose of Covid-19 Vaccine.

A.   The following documents should be produced at the time of registration for the first dose:

(i)   Student Identity Card (in original);

(ii)   National Identity Card (in original) of Responsible Party;

(iii)  Birth Certificate (recent and in original) of student;

B.  In the event that a student cannot be accompanied by her responsible party, the delegated representative (should not be a minor) should produce a letter of authorization from the responsible party to this effect.
