 Kindly find attached the amended Consent Form in respect of the second dose of COVID-19 vaccine to be administered on Wednesday 27 October 2021 at school. Please amend Paragraph D(i) of the Consent Form wherein the correct date should read Wednesday 27 October 2021 (instead of 27 November 2021)."
